 The RATE CALCULATION TABLE is used to
 define the billing charges for each
 COST RATE.

 ** 1st Dur : Amount of time at the
     beginning of each call to which a
     fixed cost is applied.(0-999sec)
 ** 1st Cst : Dollar cost for the first
     interval duration.(0-999)
 ** 2nd Dur : Amount of time for the
     duration of each billing increment
     after the first interval has
     expired.(0-999sec)
 ** 2nd Cst : Dollar cost for each
     billing increment.(0-999)
 ** Surchg  : This is a one time charge
     that is applied to the call over
     and above the time charges.
